Coordinated Veterans Care (CVC) Program

​

The CVC Program is a way for primary health care providers to assist Department of Veterans' Affairs Gold Card holders who have chronic conditions and complex health needs.

 

The CVC nurse, works closely with your doctor to provide on-going, planned and coordinated primary care, and can access community support to assist you at home.

 

Through improved community based care, the program is intended to avoid unplanned hospital admission and improve the health of participants.
