GP Chronic Condition Management Plan (GP CCMP)
A GP Chronic Condition Management Plan (GP CCMP) is a dedicated appointment with your regular GP and Practice nurse to help manage your long-term health condition. It allows you to talk through your care needs and access additional Medicare-funded support from the Australian Government if you have a chronic or terminal condition that has lasted, or is expected to last, six months or more—for example asthma, diabetes, heart disease, chronic pain, musculoskeletal conditions, cancer, stroke etc.
Eligible patients can use up to 5 discounted services per calendar year to a variety of allied health providers including (Aboriginal Health Worker, Exercise Physiologist, Podiatrist, Audiologist, Mental Health Worker, Psychologist, Chiropractor, Occupational Therapist, Speech Pathologist, Diabetes Educator, Osteopath, Dietitian or Physiotherapist).

The 5 services may be used:
- For one type of service, for example 5 physiotherapy services OR
- A combination of different types of services, for example one dietetic and 4 podiatry services.
You are eligible for these services if your medical practitioner has completed a GPCCMP.

GP Chronic Condition Management Plan Review (GP CCMP)
The GP CCMP Review is a holistic follow up appointment with your usual GP, designed to review your current health and manage your chronic condition.
During this appointment, you can discuss your ongoing care needs, receive any prescriptions you are due for, check that your immunisations are up to date, and have your blood pressure assessed.
You are eligible every 3 months to see your usual GP to review and discuss your chronic condition and this will be bulk billed.
