The Coordinated Veterans’ Care (CVC) Program supports the health and wellbeing of Gold Card veterans by providing proactive, holistic care coordination. The program focuses on improving health literacy, encouraging self-management, and connecting veterans with appropriate health services, with the aim of reducing unplanned hospital admissions through a person-centred approach.
Your GP will work in partnership with you and our DVA Practice Nurses to coordinate your primary care and develop a comprehensive care plan tailored to your health goals and treatment needs, supporting the ongoing management of your care.
This includes a Health Assessment, organising services and supports to assist you at home or in the community, and regular monthly contact either in person or by telephone.
