Health Assessments are a one-hour holistic patient centred appointment that is bulk billed through Medicare and are designed for early detection of disease and preventive health care.
Health Assessments are performed by our practice nurses working alongside your usual GP to check your blood pressure, weight and height and discuss your physical, psychological and social functioning, together with identifying any early health risk factors that may be present (for example heart disease, diabetes, or cancer). Information on health screening tools that patients may be eligible for are also discussed (for example mammograms, bowel screens, yearly blood work ups, diabetic and cardiovascular risk checkers) so that early intervention can be provided.
Health Assessments also assist seniors maintain independence and ensure appropriate support services are in place.
Eligibility:
o 75 years and older: Comprehensive, usually annual check.
o 45–49 years: At risk of developing chronic disease.
o 40–49 years: Type 2 diabetes risk evaluation.
o DVA GOLD Card Patients
o Aboriginal and Torres Strait Islander peoples
